FORDYCE ANOMALY: A CLINICAL OVERVIEW

Abstract

Fordyce anomaly is commonly known as fordyce spot. Fordyce spots are small raised bumps that appear on the lip margins, shaft of penis, labia and scrotum. They result from sebaceous glands that secrete oil to lubricate the skin and hair but are found in unlikely places. They develop as isolated bumps but may cluster together, more importantly they do not pose health risk. This review seeks to reevaluate this anomaly and create a better medical view or awareness. This treatise divulges medical knowledge for better diagnosis and prognosis. Fordyce spot may be caused by a wide range of factors from hormonal changes, to genetics, to abnormal keratinization and even aging. Fordyce anomaly is majorly characterized by a 1-3 mm sized yellowish to whitish papule. There are three main types: the oral with location on the buccal mucosa, upper and lover lip margins and rarely the tongue; the genital fordyce spot with location on the penile shaft, scrotum and rarely the labia and finally the periodical spots which may come as a major indicator of stress or hormonal imbalance. This anomaly is usually diagnosed by visual inspection and palpation as there are distinct features. Diagnosis is confirmed by ancillary investigations such as diascopy or dermatoscopy and in some cases biopsy. Biopsy, although not commonly done, might be necessary to rule out other conditions. Biopsy will reveal a well developed sebaceous gland that can be seen anywhere in the body. Fordyce spot will be characterized by enlarged sebaceous gland ducts with hyperplasia, matured sebocytes with increased sebum production of an anti-inflammatory nature as its histopathological feature. Its histochemical markers may include both cellular and sebaceous markers, while with dermatoscopy it's surface may show a smooth or slightly raised appearance and the centre appears to be brighter due to sebum build-up making this the most important feature as it completely differentiate it from other lesions, both benign and malignant. Although not a health risk, lesion may be a cosmetic concern in some people causing a need for treatment. Condition can be treated using lasers, electrosurgery, topical and cosmetic treatment. Due to this cosmetic concern and lack of societal awareness, there may be psychological and social effects g of Fordyce spot.
